Introduction
An Arteriovenous (AV) Fistula is a surgically created connection between an artery and a vein, most often in the forearm or upper arm. It is the preferred access for hemodialysis in patients with end-stage kidney disease (ESKD) because it provides strong, durable blood flow with fewer complications compared to catheters or grafts.
At Lumina Vein & Vascular Centre, our vascular surgeonoffers safe AV fistula creation and long-term care to ensure reliable dialysis access and improved patient quality of life.
Why is an AV Fistula Important?
- Provides high blood flow needed for effective dialysis
- Reduces the risk of infection and clotting compared to catheters
- Lasts longer than other access options (up to years with proper care)
- Promotes faster healing and better long-term outcomes
Indications for AV Fistula
- Chronic Kidney Disease (CKD) patients requiring regular dialysis
- Patients preparing for long-term hemodialysis access
- Rarely, AV fistulas may occur abnormally due to trauma, congenital malformations, or vascular conditions
How an AV Fistula is Created
- Performed under local or regional anesthesia
- A vein is surgically connected to a nearby artery (commonly in the wrist or elbow)
- Increased arterial blood flow causes the vein to enlarge and strengthen, making it suitable for repeated dialysis needle access
- The fistula usually takes 6–12 weeks to “mature” before use
Signs & Symptoms of an Abnormal or Complicated AV Fistula
- Swelling or bulging vein
- Pain, warmth, or redness at the site
- Numbness or tingling in the fingers (Steal syndrome)
- Blood clot formation (thrombosis)
- Bruit (whooshing sound) and thrill (vibration) absent or reduced – may indicate blockage
Diagnosis
- Physical examination: Checking for thrill and bruit
- Doppler Ultrasound: Evaluates blood flow and patency
- Angiography: For complex cases or suspected complications
Treatment & Care
- Surgical creation and repair by a vascular specialist
- Endovascular interventions (angioplasty, stenting) in blocked or narrowed fistulas
- Regular monitoring to ensure good function
- Patient education for fistula care:
- Keep the site clean and dry
- Avoid blood pressure measurement or blood draws on that arm
- Check for thrill daily
- Report swelling, pain, or bleeding immediately
